The Prize Draw is open to all current students of the Jon Jepson Black Belt Academy, Burton on Trent whose account is in good standing with no subscription fees owed. For students under the age of 18 a parent or guardian should complete the form on their behalf.
Only one entry per student is permitted. Relatives and staff of the Martial Arts Academy are excluded from entering.
The prize is a voucher valued at £20 that can be redeemed at a future belt grading. No cash alternative is available. The voucher must be redeemed within 6 months following the date of the prize draw. The date up to which the voucher is valid will be stated clearly on the voucher itself.

Incomplete or spoiled entries will not be entered in to the draw. Examples of incomplete or spoiled are if all answers to multiple choice are “prefer not to say” and/or if free text answers are not relevant or unhelpful in nature.

The draw will take place on: 20th May 2017. The first Little Ninja to arrive and pay for their belt grading on that day will be asked to make the draw.
The likelihood of you winning this prize is directly proportionate to the number of entries we receive. The maximum number of entries is equal to the number of fully subscribed members of the academy at the time the prize draw is held.
The winner will be notified by email and their name will be displayed at reception for 14 days after the draw or until they acknowledge receipt of notification of their win. We reserve the right to publish the winner’s name in traditional and electronic print for use in future promotions.
